2019 Tower Health Community Health Needs Assessment

Community Health Needs Assessments (CHNA) are required of tax-exempt hospitals as a result of the Patient Protection and Affordable Care Act. These assessments, done every three years, create an important opportunity to improve the health of communities. Working alongside community stakeholders, we can and will use the CHNA data to develop programs that meets the needs of our community.

Our 2019 CHNA identified the following key health issues: 

  • Access to Healthcare Services
  • Social Determinants of Health
  • Disease Prevention and Management
  • Behavioral Health

By November 2019, an Implementation Plan will be created. The purpose of this plan is to create measurable and strategic initiatives that address the identified key health issues.
