Disease-Specific Certification for Heart Failure Program
Certification processes are rigorous and focused on patient safety and better-quality patient outcomes. To meet the standards for this certification, a facility must meet performance measure requirements for six different measures including beta-blocker therapy, post-discharge appointments for heart failure patients, care transition record transmitted, discussion of advance directive/advance care planning, advance directive executed and post-discharge evaluation for heart failure patients.


Get With The Guidelines® Gold Plus Performance Achievement Award for Heart Failure
Hospitals receiving the Get With The Guidelines Gold Plus Achievement Award have reached an aggressive goal of treating heart failure patients with 85% or higher compliance to core standard levels of care as outlined by the American Heart Association for 24 consecutive months.
